Some teeth are past saving: cracked below the gumline, loosened by gum disease, or a wisdom tooth without room to come in straight. When that is the honest answer we say so, numb the area thoroughly, and remove the tooth as gently as the situation allows.
Most extractions are done in a single visit, and you leave with plain written instructions for the first few days of healing. If the gap should later be filled with a bridge or another option, we talk that through before anything comes out — never after.
